Participation Waiver & Release of Liability

AGREEMENT:

1. That I am participating in classes, workshops, bodywork sessions, and/or River Room sessions offered by DustOff Yoga Studio, LLC, during which I will receive instruction and services in yoga, meditation, holistic care, and wellness-based technology. I recognize that yoga and bodywork may involve physical exertion, which can be strenuous and may cause injury, and that River Room services may involve sensory stimulation or neuro-sensory devices. I am fully aware of the risks and hazards involved.

2. I understand that it is my responsibility to consult with a physician prior to and regarding my participation in studio classes, workshops, bodywork, and River Room sessions. I warrant that I am physically, mentally, and emotionally fit for participation, and I have no medical or neurological condition(s) which would prevent my full and safe involvement.

3. In consideration of being permitted to participate in the services offered by DustOff Yoga Studio, LLC, I agree to assume full and complete responsibility for any risks, injuries, or damages, known or unknown, that I or my family may incur. This includes participation in yoga, bodywork, and River Room sessions. I, on my own behalf and on behalf of my heirs, agents, representatives, and assigns, agree to hold harmless and indemnify DustOff Yoga Studio, LLC, its employees, instructors, contractors, and affiliates from any such claims, injuries, damages, or legal expenses. 4. I, on behalf of myself and my heirs, agents, and representatives, forever release, waive, and discharge DustOff Yoga Studio, LLC, its employees, instructors, contractors, and agents from any and all liability for injury, death, or other damages arising from my participation, including those resulting from negligence.

River Room & Wellness Technology Acknowledgment

The River Room at DustOff Yoga Studio is a private Relaxation & Integration Suite designed to support nervous system care and emotional well-being through the use of non-invasive sensory tools and advanced wellness technologies. These may include BrainTap, Sensate, NeuroVizr, Mendi, TouchPoints, Pulsetto, and similar tools involving audio-guided meditation, gentle vibration, visual stimuli, and breath or heart rate training.

I understand and agree that:

• River Room services are not medical in nature and do not replace professional medical, psychological, or psychiatric care.

• I am responsible for disclosing any health conditions (such as epilepsy, heart disease, unmanaged mental illness, pregnancy, or photosensitivity) and have obtained clearance from a healthcare provider if applicable.

• I may discontinue a session at any time and will use the space and equipment respectfully and as instructed.

• I voluntarily assume all responsibility and risk for participation in River Room sessions and release DustOff Yoga Studio, LLC from all liability related to such use.

General Policies & Health Guidelines

• Scheduling and content of activities may change on occasion due to unforeseen circumstances.

• A $50.00 fee will be charged for any returned checks (N.S.F.).

Health & Illness Policy:

I agree not to attend any in-person sessions, classes, or events at DustOff Yoga Studio, LLC if:

1. I have had close contact with a confirmed case of COVID-19 (or other contagious illness).

2. I am experiencing symptoms such as cough, sore throat, shortness of breath, or fever.

3. I have had a fever in the past 48 hours.
